A. You will need to bring the following items:

• Full Name
• Date of Birth
• Place of Birth
• Social Security Number
• Parents Names: (mother’s maiden name)
• Last Grade Completed
• Occupation
• Recent Photo
• Military Papers
• Clothing (ie: full undergarments, slacks, shirt with tie or dress)

A. We have the ability to ship within and across the United States.

A. Yes, we will coordinate with the military honors program for you.
